Relvay makes a pregnancy pillow. We also tell you below when you should not buy one.
The short answer
Shoulder or arm discomfort with a pregnancy pillow often comes from where your head, bottom shoulder and arms rest, not the bump support. Map the contact points and when it happens before you change pillows.
- Try first: level head pillow, bottom arm in front, top arm hugging a pillow.
- Tell your midwife about numbness or weakness that lasts.
- Urgent: sudden swelling with headache or vision changes.
3:25 a.m., and my arm was asleep again
Many side sleepers fix their bump and back support and then find a new ache higher up, in the shoulder they lie on or the arm that has nowhere to go. This is how it can feel.
3:25 a.m.
I was 30 weeks. My hips were finally okay because of the big C pillow. But I woke up again with my left arm dead from the shoulder down, pins and needles all the way to my fingers.
I sat up and shook my hand out in the dark. The top part of the pillow had been under my head and over my arm at the same time, so my shoulder was jammed and my arm was pinned under it. My neck felt crooked too.
Andre slept on. I sat on the edge of the bed scrolling for shoulder pillows at 3:30 in the morning, wondering if I was about to buy my fourth pillow of this pregnancy.
- If you wake up with the arm you lie on numb or tingling...
- If your bottom shoulder aches after an hour on one side...
- If your top arm hangs forward with nowhere to rest...
- If you are about to buy one more pillow without knowing what is wrong...
You did not set it up wrong. Most pregnancy pillows are designed around the bump, the back and the knees. The shoulder and arm are left to figure it out.

What most of us try first
Most people change the head pillow, then tuck the arm somewhere, then think about a new pillow. Each change moves the pressure without telling you where it came from.
- A thicker head pillow. The shoulder felt freer for a night. Then my neck started aching from being tipped up.Higher is not the same as level.
- Arm under the pillow.Felt cosy at bedtime. By 2 a.m. it was the arm that woke me.An arm under weight for hours goes numb.
- Arm under my cheek. My wrist was bent right back. My fingers tingled by morning.A bent wrist all night is a pressure point too.
- Switching sides more often. It spread the ache across both shoulders and woke me twice as much.Turning more is not the same as resting.
- Resting the top arm on the bump part of the pillow. It slid off the curve as soon as I relaxed, and the arm dangled over my bump again.Bump support is shaped for the bump, not the arm.

Why can a pregnancy pillow make your shoulder or arm sore?
Because on your side, your bottom shoulder carries weight for hours, your head height decides the angle of your neck, and your arms need somewhere to rest. A big pillow can change all three while it supports your bump.
Head heightToo high or too low tips your neck and loads the shoulder. Full-body pillows with a head section can change your height.
Bottom shoulder and armIf the arm ends up under you or under the pillow, it carries pressure all night.
Top arm and wristWith nothing to rest on, the top arm hangs forward. A hand tucked under the cheek bends the wrist.

Some arm and hand symptoms in pregnancy have causes no pillow changes. The NHS lists numb hands, pins and needles and a weak grip as signs of carpal tunnel syndrome, usually worse at night, with pregnancy a known risk factor. That is something to mention to your midwife or doctor, not to solve with bedding. Write those symptoms down for your appointment rather than hoping the next pillow sorts them out.
Before you change a pillow, you need to know which point is pressing and when. Without that, the next pillow is a guess.
That is how Desiree stopped guessing. First the contact-point map, then which signs go to your care team, then what changed.
How do you record the contact points?
For three nights, note five contact points and when symptoms show up. It takes two minutes in the morning and gives your care team something clear to look at.
| Contact point | What to note |
|---|---|
| 1. Head and neck | Which pillow is under your head, and whether your neck feels level, tipped up or tipped down. |
| 2. Bottom shoulder | Is it under you, slightly forward, or pinned by a pillow? |
| 3. Bottom arm | In front of you, under your head, or under the pillow. |
| 4. Top arm | Resting on something, or hanging forward over the bump. |
| 5. Hands and wrists | Straight, bent back under your cheek, or curled tight. |
Then add timing: does it start within the hour, wake you at night, or still feel numb or weak an hour after you get up? Is it one side or both hands? A photo of how you wake up, taken by your partner, is worth more than a description.
When is it more than a comfort problem?
Tell your midwife or doctor if numbness, tingling, weakness or pain lasts into the day, keeps getting worse, or makes it hard to grip. Get urgent help for sudden swelling with a severe headache, vision changes or pain below the ribs.
- Comfort change, likely: eases within minutes of moving, only after long stretches on one side, better when the arm is free.
- Raise it at your next visit or sooner: tingling in both hands most nights, a weak grip, symptoms that last after you are up.
- Call promptly: pain that spreads, weakness that affects daily tasks, anything new that worries you.
- Urgent, call your doctor or maternity unit now: sudden swelling of the face, hands or feet, a severe headache, blurred vision or flashing lights, or pain below the ribs. The NHS lists these as possible signs of pre-eclampsia.
This guide does not diagnose shoulder or arm symptoms. A pillow is a comfort aid. It should never delay getting checked.
How can you ease shoulder pressure tonight without buying anything?
Keep your head level, bring the bottom shoulder slightly forward, and give both arms somewhere to rest. Use pillows you already have.
- Use a head pillow that keeps your neck level with your spine, not tipped up.
- Ease the bottom shoulder slightly forward so you are not lying straight on its point.
- Keep the bottom arm in front of you, not under your head or under any pillow.
- Hug a regular pillow so the top arm rests at shoulder height instead of hanging.
- Keep wrists straight, not bent back under your cheek.
- Keep bump, back and knee support below your shoulders, so nothing presses on the upper arm.
Use your map for three more nights. If one point changed and the ache eased, you have found your cause, and you may not need anything new.
Which pregnancy pillow shapes leave room for your shoulder?
Shapes that support the bump and back without a head section usually leave your head pillow and arms as they were. Full-body shapes add support but also change head height and where the arms can go.
| U pillow | C pillow | Single wedge | Two wedges strapped together | |
|---|---|---|---|---|
| Changes your head height | Often, has a head section | Often | No | No, keep your own head pillow |
| Room for the bottom arm | Can be pinned | Can be pinned | Yes | Yes |
| Somewhere to rest the top arm | Yes, the front arm | Yes | No, add a pillow | No, add a pillow to hug |
| Bump and back support | Yes | Yes | One or the other | Both |
No shape fixes a shoulder by itself. Match the shape to what your map showed.
What changed when the shoulder had room?
Once nothing sat under her head or over her arm, Desiree's shoulder woke her far less, and the hand symptoms she still had went to her midwife.
Desiree kept her map for three nights. The pattern was plain: the head section of her C pillow was lifting her neck and pinning her bottom arm. She also had tingling in both hands most mornings, so she told her midwife, who suggested wrist splints at night.
She went back to her own flat head pillow, hugged a regular pillow for her top arm, and moved bump and back support to two small wedges joined by a strap. Within the week she was waking for the bathroom, not for a dead arm. On Sunday Andre found her asleep at 8 a.m. for the first time in a month.

Frequently asked questions
What is the best pregnancy pillow for shoulder pain?
There is no single best one. Shapes that support your bump and back without a head section let you keep a level head pillow and free arms. Map where your shoulder and arm rest first, then choose.
Why does my arm go numb when I sleep on my side pregnant?
Often because the arm is under your body, your head or a pillow for a long time. Tingling in the hands that is worse at night can also be carpal tunnel syndrome, which is common in pregnancy. Tell your midwife or doctor.
Should I sleep with my arm under the pillow?
Many people find it goes numb that way. Keep the bottom arm in front of you, and hug a pillow with the top arm so it rests at shoulder height.
When is arm numbness in pregnancy urgent?
Get urgent help for sudden swelling of the face, hands or feet with a severe headache, vision changes or pain below the ribs. Tell your care team about numbness or weakness that lasts or gets worse.
Does sleeping on the other side help shoulder pain?
Switching sides spreads the load, and the NHS says either side is fine in pregnancy. If both shoulders ache, look at head height and arm position first.
Who can help with shoulder pain in pregnancy?
Start with your midwife or doctor. If the pain keeps coming back or limits what you can do, they may refer you to a physiotherapist who works with pregnant people. Bring your contact-point map.
Can a pillow fix carpal tunnel in pregnancy?
No. Resting your hands and keeping wrists straight may feel better, and NHS leaflets mention elevation and night splints. Ask your midwife or doctor what suits you.
