Early in pregnancy, yes, if it feels comfortable. Tommy's says stomach sleeping is fine in the early months. As your bump grows it usually becomes too uncomfortable, and from 28 weeks the advice is to go to sleep on your side.

Is it okay to sleep on your stomach in early pregnancy?

Yes. Tommy's says it is fine to sleep on your front in the early part of pregnancy. In the first trimester your womb is still small and sits low in your pelvis, so for most people the limit is comfort, not safety.

The first thing that often changes is not the bump at all. Tender, sore breasts can make lying face down feel bad weeks before anyone can tell you are pregnant.

I have slept on my stomach since I was a kid. Face in the pillow, one knee hitched up, arm under my head. At 17 weeks that position just quietly stopped working. It felt like lying on a small, firm ball.

I would start on my side because I was trying to be good, then wake up half turned onto my front without remembering how I got there. I did not know if that was a problem or just my body looking for the only position it knew.

Losing your favorite sleep position is a real loss. It is fine to be annoyed about it.

What changes in the second and third trimesters?

Your bump grows forward, and lying on it becomes uncomfortable for most people somewhere around the middle of pregnancy. By the third trimester, Tommy's notes a large bump makes stomach sleeping very unlikely.

From 28 weeks, UK guidance is to go to sleep on your side, for night sleep, for going back to sleep after waking and for naps. If you find yourself on your front, Tommy's says to simply roll onto your side.

You will see stronger claims online about what stomach sleeping does later in pregnancy. Most of them come without a source. If you are worried about something specific, that is a question for your midwife or OB-GYN, not for a search box at midnight.

What position feels closest to stomach sleeping?

Many former stomach sleepers like a three-quarter position: on your side, rolled slightly forward, top knee bent and resting on a pillow, hugging something in front of you. It gives you some of that pressed-in feeling while your bump stays supported.

  1. Start on your side. Either side.
  2. Bring your top knee forward and rest it on a firm pillow so your hip does not twist down to the mattress.
  3. Put a soft pillow under your bump so it feels held rather than hanging.
  4. Hug a pillow against your chest. This is the part stomach sleepers usually miss most.
  5. Tuck a pillow behind your back so you do not slowly roll the other way.

Regular pillows are enough to test this. Try it for a few nights before you decide you need anything new.

What about pillows with a hole for your bump?

Read exactly what the maker says the product is for, then ask your care team whether that fits your pregnancy. A product being sold does not tell you it suits your week or your body.

Some cushions and inflatable mats are designed so you can lie face down with a space for your bump. They are often sold for massage or short rests rather than a whole night. Check the stated intended use, weight and week limits, and what the instructions say about sleeping.

When should stomach sleeping become a call to your midwife or doctor?

Position alone is rarely an emergency. Call your care team if you have pain, bleeding, fluid loss, or your baby's movements feel different from usual, whatever position you were in.

  • Pain in your bump that does not ease when you change position.
  • Bleeding or leaking fluid.
  • Your baby's movements feel slower or different from their usual pattern. Call your maternity unit that day, at any hour.
  • You cannot get comfortable in any position and it is wrecking your sleep for weeks.
